What is Choreoathetosis?

Choreoathetosis is a hyperkinetic condition arising from dysfunction in basal ganglia circuits, in which rapid and migrating choreiform movements are seen together with slow, twisting athetoid movements.

Etiology includes Huntington disease, cerebral palsy, postanoxic injury, drug-induced dyskinesias, autoimmune and metabolic disorders. Paroxysmal forms also exist.

Diagnosis is made with clinical observation, family history, imaging, and genetic studies when needed. Treatment is determined according to the type of cause.

Symptoms

Rapid, irregular jerking movements (chorea)
Slow, writhing-type movements (athetosis)
Unsteady gait and falls
Difficulty with fine motor skills
Speech and swallowing problems
Mood changes and behavioral disturbances
Fatigue and muscle tension

Risk Factors

Family history of Huntington disease
Perinatal hypoxia and cerebral palsy
Rheumatic fever and Sydenham chorea
Pregnancy (chorea gravidarum)
Autoimmune diseases (SLE, antiphospholipid syndrome)
Use of drugs such as levodopa and antipsychotics
Metabolic/endocrine disorders

  • If uncontrollable involuntary movements have begun
  • If there is family history and symptoms have developed
  • If there is a risk of falls and injury
  • If cognitive and mood changes have been added
  • If movement disorder has emerged after drug use
