What is Manual Therapy in Cervicogenic Headache?

Cervicogenic headache (CGH) is a headache arising from the upper cervical region, referred from the C1–C3 joints and soft tissues to the trigeminocervical nucleus. IHS and Sjaastad criteria are used in diagnosis. It is unilateral, does not fluctuate, and is triggered by neck movements.

Manual therapy approaches: 1) upper cervical Maitland mobilization (C0–C3), 2) Mulligan SNAG (sustained natural apophyseal glide), 3) Grade IV mobilization or manipulation, 4) suboccipital release, 5) myofascial release (upper trapezius, levator scapulae).

Exercise therapy: craniocervical flexor training (CCFT), deep neck flexor strengthening, posture correction (scapular setting), stretching exercises. Randomized trials have shown that combining manual therapy and exercise is superior to medication alone.

Symptoms

Unilateral occipital/frontal headache
Pain triggered by neck movement
Restricted neck mobility
Tenderness of the upper cervical region
Pain that increases with position
Photophobia/phonophobia without spread
