What is Cerebellar Ataxia — Balance Rehabilitation?

Cerebellar ataxia is impairment of motor planning and coordination because of damage to cerebellum or cerebellar pathways.

Causes are degenerative (SCA, Friedreich), vascular (cerebellar stroke), demyelinating (MS) and toxic (alcohol, phenytoin).

Loss of intentional movement smoothness, gait widening (wide-based gait), dysmetria, intention tremor and dysdiadochokinesia are findings.

Symptoms

Wide-based unsteady gait (ataxic gait)
Hand-foot dysmetria and intention tremor
Dysarthria (slurred speech)
Nystagmus and visual disturbance
Sitting and standing balance problem
Difficulty in fine motor skill
Difficulty climbing-descending stairs

Risk Factors

Hereditary cerebellar ataxia (SCA1-50, Friedreich)
Multiple sclerosis and demyelinating disease
Cerebellar stroke and tumour
Alcohol-related cerebellar atrophy
Phenytoin and chronic anticonvulsant use
Vitamin B12 and vitamin E deficiency
Paraneoplastic cerebellar degeneration

When to See a Doctor?

If you experience any of the following symptoms, seek medical attention promptly:

  • If sudden ataxia develops, urgent neurological evaluation is mandatory (stroke risk)
  • Definite genetic counselling is essential in any progressive ataxia
  • Brain MRI and lumbar puncture are required
  • Multidisciplinary rehabilitation is important for functional decline

Treatment Methods

01
Bobath and Frenkel exercises (balance and coordination)
02
Static and dynamic balance training
03
Visual feedback (mirror, Wii balance board)
04
Proprioception training and gait training
05
Walker, cane and orthotic support
06
Buspirone, riluzole (limited evidence)
