What is Cemento Osseous Dysplasia?

Cemento osseous dysplasia is a non neoplastic process in which normal jaw bone is replaced by fibrous tissue cementum like and bone like material. The condition has three subtypes including periapical focal and florid forms. Periapical form most commonly involves the apices of mandibular anterior teeth. The condition is more frequent in middle aged women and individuals with darker skin pigmentation reflecting genetic and hormonal influences on its development.

Lesions evolve through three stages including early radiolucent stage suggesting periapical pathology mixed radiolucent radiopaque intermediate stage and mature predominantly radiopaque stage. Adjacent teeth maintain vitality differentiating cemento osseous dysplasia from periapical inflammatory lesions. Imaging characteristics on panoramic and periapical radiographs allow diagnosis without biopsy in typical cases.

Management is conservative since lesions are asymptomatic and benign. Vitality testing of adjacent teeth confirms diagnosis distinguishing the condition from periapical pathology. Endodontic or surgical intervention is contraindicated as the lesions are avascular and prone to secondary infection if exposed. Periodic radiographic monitoring tracks lesion stability. Florid form may rarely become symptomatic with secondary infection requiring antibiotic therapy and possible surgical curettage. Patient education prevents unnecessary procedures and reassures regarding benign nature.
