What is Carotid Doppler Ultrasound?

Carotid Doppler ultrasonography is a non-invasive, radiation-free imaging method that provides anatomical and hemodynamic evaluation of the internal and external carotid arteries. Plaque morphology is evaluated with B-mode, flow direction with color Doppler, and flow velocities with spectral Doppler.

Stenosis degree is calculated with NASCET and ECST criteria: peak systolic velocity (PSV) >125 cm/s indicates >50% and PSV >230 cm/s indicates >70% narrowing. Intima-media thickness (IMT) shows subclinical atherosclerosis.

It is used in carotid evaluation after stroke and TIA, asymptomatic carotid bruit, screening in patients with coronary artery disease, and follow-up before-after carotid endarterectomy/stenting.

Symptoms

Having had a TIA or stroke
Carotid bruit on auscultation
Coronary artery disease (accompanying carotid stenosis)
Peripheral arterial disease
Presence of cardiovascular risk factors

Risk Factors

No specific risk for the examination (non-invasive and radiation-free)
Operator dependence (requires experienced radiologist)
Heavily calcified plaque can reduce ultrasound penetration
Technical difficulty in tortuous or high-bifurcation arteries

When to See a Doctor?

If you experience any of the following symptoms, seek medical attention promptly:

  • Urgent evaluation after experiencing a TIA or stroke
  • When carotid bruit is detected on physical examination
  • In follow-up of known carotid stenosis
  • Before coronary bypass or major vascular surgery
