What is Carotid Artery Stenosis?

The carotid arteries are among the main vessels feeding the brain. As a result of atherosclerosis, plaques composed of fat, calcium, and other substances accumulate in these vessels, narrowing the vessel lumen; this condition is called carotid stenosis.

Carotid stenosis usually progresses without symptoms and is detected by a bruit heard on neck examination or by imaging tests. Fragments that break off from the plaque can travel to the brain, leading to TIA or stroke.

The degree of stenosis and the presence of symptoms determine the treatment plan. In moderate-to-severe stenosis, interventional treatment may be considered in addition to lifestyle changes and medical therapy.

Symptoms

Mostly asymptomatic
Transient vision loss (amaurosis fugax)
Sudden arm or leg weakness
Difficulty speaking
Vascular bruit heard in the neck (on examination)
TIA or stroke clinical picture

Risk Factors

Hypertension and high LDL cholesterol
Diabetes mellitus
Smoking
Advanced age and male sex
Family history of vascular disease

When to See a Doctor?

If you experience any of the following symptoms, seek medical attention promptly:

  • Emergency consultation should be made when TIA or stroke-like symptoms occur
  • When vascular bruit is heard in the neck
  • For routine screening in individuals with high risk factors
  • If bilateral carotid stenosis is suspected

Treatment Methods

01
Antiplatelet drugs (aspirin, clopidogrel)
02
Statin therapy with plaque stabilization
03
Blood pressure and diabetes control
04
Carotid endarterectomy in symptomatic severe stenosis
05
Carotid stenting in patients with high surgical risk
