What is Cardiac Rehabilitation — Phase 3 Long-Term Follow-up?

Cardiac rehabilitation is divided into 4 phases: phase 1 (in-hospital, 1–7 days), phase 2 (outpatient, 6–12 weeks), phase 3 (community/home, 6–12 months), phase 4 (lifelong).

Phase 3 is a programme that takes 6–12 months which the patient continues at home or in a community centre after the outpatient programme.

It includes regular exercise, dietary education, smoking cessation, weight management and medication compliance follow-up.

Symptoms

Conditions in which it is targeted: post-myocardial infarction
Post-coronary bypass and stent
Stable angina pectoris
Heart failure (NYHA II-III)
Post-cardiac valve surgery
Post-heart transplantation
Peripheral arterial disease (PAD)

Risk Factors

Long-term programme compliance is variable in cardiac patients
Sedentary lifestyle and obesity continuation risk
Smoking restart risk
Depression and anxiety affect compliance
Distance to centre and lack of social support
Low income and educational level
Lack of family/spouse support

When to See a Doctor?

If you experience any of the following symptoms, seek medical attention promptly:

  • Routine cardiology follow-up every 3–6 months is essential
  • Stress test repetition is required for chest pain or exercise intolerance
  • Definite evaluation must be made for arrhythmia and palpitation symptoms
  • Annual lipid profile, HbA1c and BNP follow-up are recommended
