What is Cardiac Rehabilitation: Phase Programs?

Cardiac rehabilitation is a comprehensive secondary prevention program combining supervised exercise training, risk factor modification, education, and psychosocial support.

Phase 1 (inpatient): begins within 24-48 hours of stable cardiac event, focuses on early mobilization, basic education, and discharge planning.

Phase 2 (outpatient supervised): 8-12 weeks of monitored exercise (3 sessions/week), individualized exercise prescription, comprehensive risk reduction, typically 36 sessions.

Phase 3 (maintenance/lifelong): community-based or home-based program emphasizing long-term lifestyle changes, exercise adherence, and continued risk factor management.

Symptoms

Recent myocardial infarction, percutaneous coronary intervention, or coronary artery bypass grafting
Heart failure with reduced or preserved ejection fraction (NYHA II-III)
Heart valve surgery, heart transplantation
Stable angina, peripheral arterial disease, claudication
High cardiovascular risk profile requiring intensive lifestyle modification

Risk Factors

Unstable angina, decompensated heart failure, severe aortic stenosis (need stabilization first)
Recent uncontrolled arrhythmia, severe orthostatic hypotension
Active myocarditis, pericarditis, or endocarditis
Severe pulmonary hypertension, recent pulmonary embolism
Advanced cognitive impairment limiting program participation
