What is Cardiac CT Angiography (Detailed)?

Cardiac CT angiography (CCTA) is a comprehensive non-invasive imaging modality that uses ECG-gated multi-detector CT with iodinated contrast to evaluate coronary arteries, cardiac chambers, valves, pericardium and great vessels in a single examination.

Modern systems with 256-640 slice scanners provide isotropic submillimeter resolution and temporal resolution below 75 ms, allowing motion-free imaging of coronary arteries even at higher heart rates while keeping radiation dose below 5 mSv with iterative reconstruction.

Symptoms

Atypical or stable chest pain in low-to-intermediate risk patients
Inconclusive or equivocal stress test results
Suspected coronary anomaly or fistula
Pre-operative coronary mapping for valve or aortic surgery
Acute chest pain triple rule-out (ACS, PE, aortic dissection)
Cardiac mass, thrombus or pericardial pathology evaluation

Risk Factors

Iodinated contrast hypersensitivity
Severe renal impairment (eGFR <30)
Uncontrolled tachycardia or arrhythmia
Morbid obesity exceeding scanner limits
Pregnancy (relative contraindication)
Inability to perform breath-hold or remain still
Severe coronary calcification (Agatston >1000) reducing accuracy

When to See a Doctor?

If you experience any of the following symptoms, seek medical attention promptly:

  • Stable chest pain with intermediate pretest probability of CAD
  • Equivocal stress imaging or symptoms despite optimal therapy
  • Pre-TAVI or pre-CABG anatomical evaluation
  • Suspicion of coronary anomaly in young athletes
  • Aortic dissection or pulmonary embolism rule-out in ED
  • Cardiac mass detected on echocardiography
