What is CADASIL?

CADASIL (cerebral autosomal dominant arteriopathy with subcortical infarcts and leukoencephalopathy) is a hereditary disease affecting small cerebral vessels caused by NOTCH3 gene mutations.

It presents in middle age with migraine attacks with aura, recurrent subcortical strokes, mood disorders, and progressive cognitive decline. A similar history in different family members is striking.

Diagnosis includes characteristic temporal pole white matter findings on MRI, NOTCH3 genetic testing, and electron microscopic findings on skin biopsy.

Urinary incontinence
Pseudobulbar features

Risk Factors

Familial inheritance (autosomal dominant)
NOTCH3 mutation
Cardiovascular risk factors (smoking, hypertension)
Equal effect on women and men
History of stroke at a young age
Unexplained white matter lesions
Young adults with migraine
