What is Brandt–Daroff Exercises?

Described by Thomas Brandt and R. Daroff in 1980, these are home-based positional exercises for BPPV and other positional vertigos that the patient can perform unsupervised.

The patient starts seated, lies onto one side, waits 30 seconds, returns to sitting, and then lies onto the opposite side. One set comprises five repetitions; three sets per day are recommended.

They work more slowly than the Epley or Semont manoeuvres, with symptoms typically improving within 1–2 weeks. Exercises may worsen vertigo initially but adaptation develops with repetition.

Symptoms

Recurrent BPPV episodes
Positional vertigo
Failure of Epley or Semont manoeuvres
Mild positional dizziness
Sense of imbalance
Need for vestibular adaptation

Fear of head movement
Need for vestibular rehabilitation

When to See a Doctor?

If you experience any of the following symptoms, seek medical attention promptly:

  • When home exercises are requested for BPPV
  • For reinforcement after a clinical manoeuvre
  • For mild positional vertigo
  • For vestibular hypofunction
  • To assess exercise tolerance

Treatment Methods

Sit on the edge of the bed

Turn the head 45° away and rapidly lie to that side

Wait 30 seconds (until symptoms subside)

Return to sitting for 30 seconds

Perform the same movement on the opposite side

5 repetitions, 3 sets per day

Continue regularly for 2 weeks

Reduce the dose once symptoms are controlled
