What is Bobath Concept (Neurodevelopmental Treatment)?

The Bobath concept, also known as Neurodevelopmental Treatment (NDT), was developed by Berta and Karel Bobath in the 1940s for rehabilitation of central nervous system disorders. It uses a problem-solving approach focusing on individualized assessment, hands-on facilitation of normal movement patterns and integration of postural control with selective movements.

Modern Bobath emphasizes contemporary motor learning theory, neuroplasticity, task-specific training and active engagement of the patient. Therapists use specific handling techniques and key points of control (head, shoulders, pelvis) to facilitate efficient movement, normalize tone, improve alignment and prevent compensatory strategies that could lead to long-term impairments.

Indications include adult hemiplegia after stroke, traumatic brain injury, cerebral palsy in children, multiple sclerosis and other neurological conditions affecting motor control. The approach is integrated with task-oriented training, constraint-induced movement therapy, robot-assisted rehabilitation and gait training. Outcomes assess functional independence, postural control, gait quality and quality of life.

Symptoms

Hemiplegia or hemiparesis after stroke
Spasticity
Loss of selective movement
Postural control deficits
Gait abnormalities
Difficulty with sit-to-stand transitions
Upper limb weakness and motor planning issues
Cerebral palsy motor delays
Imbalance and falls risk
Loss of independence in daily activities

Risk Factors

Stroke (ischemic or hemorrhagic)
Traumatic brain injury
Cerebral palsy
Multiple sclerosis
Spinal cord injury
Late presentation delaying rehabilitation
Cognitive impairment limiting motor learning
Comorbid orthopedic conditions
Severe spasticity
Caregiver fatigue limiting home practice

When to See a Doctor?

If you experience any of the following symptoms, seek medical attention promptly:

  • New-onset weakness or stroke symptoms
  • Worsening of motor function
  • Severe spasticity affecting daily care
  • Falls during rehabilitation
  • Pain or contracture development
  • Suspected neurological deterioration
  • Need for orthosis or assistive device
  • Pediatric motor developmental delays
