What is Aquatic Therapy — Pool-Based Rehabilitation?

Aquatic therapy (hydrotherapy, pool-based rehabilitation) is a structured therapeutic exercise program performed in a temperature-controlled pool (usually 32-34°C). It exploits the physical properties of water, namely buoyancy, hydrostatic pressure, viscosity-related resistance and warmth, to facilitate movement that is painful or impossible on land.

Buoyancy reduces compressive joint loads and supports weakened limbs, allowing earlier mobilization after orthopedic surgery and in obese patients. Hydrostatic pressure improves venous return, reduces edema and stabilizes the trunk, while heat reduces muscle tone, facilitates relaxation and improves circulation. Resistance can be graded by speed and surface area of motion.

Indications include osteoarthritis, low-back pain, post-arthroplasty rehabilitation, multiple sclerosis, stroke, cerebral palsy, fibromyalgia and ankylosing spondylitis. Contraindications include open wounds, uncontrolled cardiac failure, severe cognitive impairment, certain skin infections, and incontinence requiring containment. Programs are individualized using methods such as Halliwick, Bad Ragaz, Watsu and aquatic PNF.
