What is Aquatic Therapy in Advanced Arthritis?

Aquatic therapy in advanced arthritis is a multimodal hydrotherapy technique used to reduce joint load and increase function in patients with severe arthritis. Provides analgesic and relaxation effect with warm water (32-34°C); buoyancy reduces joint load by 50-90%; hydrostatic pressure reduces edema.

Indications: advanced knee/hip osteoarthritis (KL grade 3-4), rheumatoid arthritis (active or refractory), ankylosing spondylitis, fibromyalgia, juvenile idiopathic arthritis (JIA), psoriatic arthritis, post-arthroplasty rehabilitation. Contraindications: open wound, urinary/fecal incontinence, severe cardiovascular disease, uncontrolled epilepsy, severe respiratory disease.

Therapy protocol: pool 32-34°C, depth 1-1.5 m, 30-45 min/session, 2-3 days/week, 6-12 weeks total. Components: warm-up (5-10 min), range of motion exercises (10-15 min), strengthening (resistance with water resistance), aerobic (water walking, water aerobics), Halliwick method (10-point program), Bad Ragaz Ring Method (PNF based). Cochrane review (2014, 2016): moderate-strong evidence — pain, function, quality of life improvement.

Risk Factors

Advanced KL grade osteoarthritis (3-4)
Active rheumatoid arthritis
Body mass index >30 (high joint load)
Joint contracture
Conventional therapy intolerance
Pre-arthroplasty preparation
