Anti-Hu Antibody Paraneoplastic Sensory Neuronopathy

What is Anti-Hu Antibody Paraneoplastic Sensory Neuronopathy?

Anti-Hu (ANNA-1) antibody is the target of antibodies against an intracellular RNA-binding protein expressed in central and peripheral neurons. Anti-Hu antibodies are associated with the most classical paraneoplastic neurological syndromes.

Underlying tumor is typically small-cell lung cancer (SCLC, 90%), with a small percentage being neuroblastoma, prostate or breast cancer. Sensory ganglionopathy presents with asymmetric subacute sensory loss, ataxia, dysesthesia and pseudoathetoid movements.

Diagnosis is based on demonstration of anti-Hu antibodies in serum, intensive tumor screening (CT, FDG-PET), nerve conduction study (low-amplitude SNAP, normal CMAP) and skin biopsy. Treatment of the underlying tumor and immunotherapy are administered. Prognosis is poor.

Symptoms

Subacute asymmetric sensory loss
Sensory ataxia and gait disturbance
Severe dysesthesia and pain
Pseudoathetoid movements (hands)
Loss of proprioception
Coexisting limbic encephalitis findings

Risk Factors

Smoking and SCLC
Advanced age (60-75 years)
Male sex (slight predominance)
Other paraneoplastic syndrome history
Underlying neuroblastoma (rare)
Genetic predisposition (HLA)
