What is Advanced Vestibular Rehabilitation?

Vestibular rehabilitation therapy (VRT) treats vestibular disorders by exploiting central compensation and neuroplasticity. It is built on three principles: 1) habituation — building tolerance through repeated exposure to triggering stimuli, 2) adaptation — recalibration of vestibulo-ocular reflex (VOR) gain, 3) substitution — compensation through visual and somatosensory inputs.

BPPV is treated with the Epley (posterior canal), Semont and Lempert (lateral canal) manoeuvres and Brandt-Daroff exercises. Cawthorne-Cooksey exercises are used for unilateral vestibular loss; substitution training is the focus in bilateral loss.

VRT is also effective in post-concussion vertigo, persistent postural-perceptual dizziness (PPPD) and central vertigo. Virtual reality integration boosts engagement. Treatment usually lasts 4-12 weeks, with daily home exercises being crucial.
