What is Advanced Botulinum Toxin in Dynamic Equinus Spasticity?

Dynamic spastic equinus deformity following cerebral palsy, stroke and TBI adversely affects functional walking. Botulinum toxin A (Botox) injections produce localised relaxation in target muscles.

Ultrasound and EMG guidance is required for accurate targeting of small muscles. Frequent target muscles include the gastrosoleus complex, tibialis posterior and flexor hallucis longus.

The effect lasts 3-6 months; outcomes are improved by combining with functional exercise, Achilles stretching, casting and AFO use. Repeated administration and dose titration are required.
