What is ADEM (Acute Disseminated Encephalomyelitis)?

ADEM is an immune-mediated demyelinating disease that involves the brain and spinal cord in a multifocal manner, usually appearing within days to weeks after a viral infection or vaccination.

It classically follows a monophasic course; however, some cases can proceed with relapses and may convert to MS or MOGAD diagnoses on follow-up.

Encephalopathy (sleepiness, confusion of consciousness) is among the diagnostic criteria and is an important feature distinguishing the disease from MS.

Symptoms

Neurological findings following fever and malaise
Change in consciousness
Headache
Motor weakness
Sensory loss
Gait disorder with ataxia
Seizures

Risk Factors

Recent viral infection
Childhood
Recent vaccination (rarely)
Immune system activation
