What is Mechanical Thrombectomy in Acute Ischemic Stroke?

Acute ischemic stroke is a neurological emergency that occurs as a result of sudden occlusion of a vessel feeding the brain and must be treated under time pressure.

Mechanical thrombectomy is the procedure of accessing the occluded vessel by entering through the groin vessel with a catheter and removing the clot with a stent retriever or aspiration device.

It can be applied with or without thrombolysis in selected patients; the presence of salvageable tissue (penumbra) on brain imaging is among the most important indication criteria.

Symptoms

Sudden-onset unilateral weakness
Speech disorder (aphasia and dysarthria)
Facial droop and asymmetry
Visual field loss
Sudden balance and coordination loss
Sudden-onset double vision
Consciousness changes and confusion

Risk Factors

Hypertension and diabetes
Atrial fibrillation
Smoking and excessive alcohol consumption
High cholesterol
Obesity and sedentary lifestyle
History of previous stroke or transient ischemic attack
Carotid artery disease

When to See a Doctor?

If you experience any of the following symptoms, seek medical attention promptly:

  • Call 112 immediately when FAST criteria (face, arm, speech disorder) are positive
  • Sudden headache, imbalance, and nausea
  • Sudden double vision and balance loss
  • Suspected transient ischemic attack
  • Unexplained consciousness change
  • New neurological findings noticed upon waking

Rapid triage and emergency brain CT, CT angiography, CT perfusion imaging

Intravenous thrombolysis (tenecteplase or alteplase) for eligible patients

Mechanical thrombectomy that can extend up to 24 hours in large vessel occlusion

Thrombus removal with stent retriever and/or aspiration catheter

Blood pressure, glucose, and temperature control in neuro-intensive care

Antithrombotic therapy, secondary prevention, and risk factor management
