What is 4D Flow MRI?

4D flow MRI is a time-resolved three-directional phase-contrast technique that measures blood velocity within an entire volume across the cardiac cycle. Unlike 2D phase contrast, it provides a comprehensive map of intravascular hemodynamics.

Clinical applications include evaluation of aortic aneurysms and dissection, assessment of valvular regurgitation, congenital heart disease (Fontan circulation, transposition repairs) and pulmonary hypertension.

Postprocessing yields visualization of streamlines, pathlines and quantitative parameters such as wall shear stress, kinetic energy and pulse-wave velocity. Long acquisition times and need for skilled postprocessing remain limitations.

Symptoms

Bicuspid aortic valve with progressive aortic dilation
Postoperative congenital heart disease
Suspected intracardiac shunt
Complex valvular regurgitation
Pulmonary arterial hypertension assessment
Cerebral aneurysm hemodynamics

Risk Factors

Need for repeated cardiovascular imaging
Avoidance of ionizing radiation
Children and young adults
Pregnancy (later trimesters, with caution)
Patients unable to undergo CT angiography
