What is Therapeutic Drug Monitoring in Psychiatry?

Therapeutic drug monitoring is the measurement of medication blood levels to support dosing decisions in clinical care. In psychiatry it is best established for lithium, clozapine, valproate, carbamazepine, and tricyclic antidepressants where the relationship between dose, level, and effect is well defined.

Indications include suspected nonadherence, unexpected response or toxicity, drug interactions, organ dysfunction, pregnancy, advanced age, and genetic polymorphisms in cytochrome P450 enzymes. Trough levels drawn at steady state and twelve hours after the last dose are most informative.

Reference ranges are guides rather than rigid targets, and clinical judgment integrates levels with symptoms, side effects, and patient context. Pharmacogenetic testing of CYP2D6 and CYP2C19 helps interpret unusual results and personalize psychiatric prescribing.
