What is Synthetic Cannabinoid Use Disorder?

Synthetic cannabinoid use disorder involves problematic, compulsive use of synthetic cannabinoid receptor agonists (SCRAs) — designer drugs sprayed onto plant material and smoked, vaped, or ingested.

SCRAs (e.g., JWH-018, AM-2201, AB-CHMINACA, MDMB-CHMICA) bind cannabinoid receptors with up to 100 times the potency of THC, producing severe and unpredictable toxicity profiles.

These drugs evade traditional drug screens and frequently change chemical structure to circumvent regulation, contributing to outbreaks of severe intoxication and death.

Symptoms

Acute psychosis with paranoia, hallucinations, agitation
Severe agitation and aggressive behavior
Tachycardia, hypertension, chest pain, myocardial infarction
Seizures, status epilepticus
Acute kidney injury
Hyperthermia, rhabdomyolysis
Withdrawal: irritability, anxiety, insomnia, sweating, tremor, intense craving
Cognitive impairment, memory deficits with chronic use

Risk Factors

Adolescent and young adult use
Incarceration (synthetic cannabinoids commonly used to evade drug testing)
Pre-existing psychiatric disorders, especially psychosis-prone
