What is Suicide Prevention and Crisis Intervention?

Suicidality is a multifactorial outcome of biological, psychological and social vulnerabilities interacting with acute stressors.

Crisis intervention focuses on immediate safety, stabilization, hope-building and connection with treatment resources.

Comprehensive assessment includes ideation frequency, intensity, plan, intent, prior attempts, access to means and protective factors.

Validated tools (Columbia Suicide Severity Rating Scale, Beck Scale, Patient Health Questionnaire item 9) guide stratification.

Effective interventions include safety planning intervention, means restriction counseling, brief contact interventions, CBT for suicidality, DBT, CAMS and pharmacotherapy with antidepressants, lithium or clozapine for selected cases.

Symptoms

Direct or indirect statements about suicide, hopelessness or being a burden on others.
Withdrawal from family, friends and previously enjoyed activities.
Sleep disturbance, agitation, anhedonia, severe anxiety and intense psychic pain.
Increased substance use, reckless behavior, giving away possessions or sudden calmness after agitation.
Active planning, gathering means, writing notes or saying goodbye are urgent warning signs.

Risk Factors

Mood disorders, schizophrenia, post-traumatic stress disorder, substance use and personality disorders.
History of suicide attempt, family history of suicide, recent psychiatric hospitalization.
Recent loss, financial crisis, legal problems, relationship conflict, bullying, chronic pain or illness.
Access to firearms, medications, or other lethal means substantially increases risk.
Adolescents and older adults face specific developmental risk factors and barriers to care.

When to See a Doctor?

If you experience any of the following symptoms, seek medical attention promptly:

  • Active suicidal ideation with plan or intent requires immediate emergency evaluation.
  • Recent suicide attempt or self-harm needs urgent psychiatric care and medical management.
  • Severe agitation, psychosis, command hallucinations or impaired judgment warrant inpatient assessment.
  • Persistent suicidal thoughts despite outpatient treatment require step-up to intensive services.
  • Children, adolescents and elderly with suicidal communications need rapid specialist consultation.

Safety planning intervention identifies warning signs, internal coping skills, social distractions, contacts and crisis resources.

Means restriction counseling reduces access to firearms, medications and other lethal methods.

Brief contact interventions (caring postcards, follow-up calls, ED safety planning) significantly reduce repeat attempts.

Evidence-based psychotherapies include CBT for suicide prevention, DBT for borderline personality disorder, CAMS framework and trauma-focused therapy.

Pharmacotherapy includes optimization of mood-stabilizing or antidepressant treatment, addition of lithium or clozapine when indicated, and management of comorbid conditions; community follow-up, lethal means counseling and family involvement are essential.
