What is Schizoaffective Disorder?

Schizoaffective disorder is a psychiatric condition that shares features with both schizophrenia and mood disorders.

DSM-5-TR requires an uninterrupted period of illness during which there is a major mood episode concurrent with active-phase symptoms of schizophrenia, plus at least two weeks of psychotic symptoms without prominent mood symptoms during the lifetime illness.

Subtypes include bipolar type (with manic episodes) and depressive type (only major depressive episodes).

Course and prognosis generally lie between those of schizophrenia and mood disorders; integrated treatment significantly improves quality of life.

Symptoms

Delusions, hallucinations, or disorganized thinking consistent with schizophrenia
Major depressive episode with sad or empty mood and loss of interest
Manic episodes with elevated, irritable mood and increased activity in the bipolar type
Negative symptoms such as reduced motivation and emotional expression
Cognitive impairment affecting attention and planning
Periods of psychosis persisting even when the mood is stable

Risk Factors

Family history of schizophrenia, bipolar disorder, or schizoaffective disorder
Childhood adversity, trauma, or cumulative stress
Cannabis or stimulant use disorder
Obstetric complications or prenatal infections
Female sex for the depressive subtype
Presence of comorbid anxiety or substance use

  • Seek evaluation when persistent mood changes occur together with unusual beliefs, perceptions, or disorganized thinking.
  • Contact emergency services if there is risk of suicide, harm to others, or severe self-neglect.
  • A full medical review helps exclude substance-induced or medical causes of psychosis with mood disturbance.
