Myasthenic Crisis

What is Myasthenic Crisis?

Myasthenic crisis is a potentially life-threatening condition that arises with severe weakness of respiratory muscles in myasthenia gravis and may require mechanical ventilation.

Triggers include infections, surgery, certain medications (aminoglycosides, beta blockers), excessive fatigue, and changes in medication doses. Shortness of breath, swallowing difficulty, and slurred speech may be heralds of crisis.

Early recognition is critical; respiratory function tests (FVC measurement) help predict an approaching crisis. IVIG or plasmapheresis is administered in intensive care.

Symptoms

Increasing shortness of breath
Swallowing difficulty and aspiration risk
Progressive deterioration of speech
Worsening of eyelid drooping and double vision
Marked weakness in neck and extremity muscles
Agitation and high respiratory rate

Risk Factors

Diagnosis of myasthenia gravis
Acute infection
Discontinuation or dose change of anticholinesterase medication
Surgery or anesthesia
Some antibiotics and cardiac medications

When to See a Doctor?

If you experience any of the following symptoms, seek medical attention promptly:

  • Sudden difficulty breathing in a patient with myasthenia gravis — call 112 immediately
  • If swallowing or speech disturbance is rapidly worsening
  • If neuromuscular symptoms increase along with fever
  • If FVC values are dropping rapidly (during monitoring)
