What is Idiopathic Intracranial Hypertension?

Idiopathic intracranial hypertension (pseudotumor cerebri) is a condition characterized by increased intracranial pressure without any structural lesion. It is particularly common in overweight young women.

Clinically, chronic headache, blurred vision, double vision, pulsatile tinnitus and visual field loss occur. Papilledema is the typical finding on fundus examination.

Diagnosis is made with brain MRI (to exclude lesions) and lumbar puncture showing high opening pressure. Untreated, it can lead to permanent vision loss.

Symptoms

Severe, progressive headache
Blurred vision and visual field loss
Double vision
Pulsatile tinnitus (pulsing sound in the ear)
Nausea and vomiting
Headache worsening with cough or bending
Rarely, facial pain

Risk Factors

Overweight and obesity
Female sex
Reproductive age
Rapid weight gain
Some drugs (vitamin A, tetracyclines, some contraceptives)
Polycystic ovary syndrome
Sleep apnea

When to See a Doctor?

If you experience any of the following symptoms, seek medical attention promptly:

  • If there is progressive severe headache
  • If there is blurred vision or transient visual loss attacks
  • When papilledema is detected
  • If double vision develops
  • New-onset severe headache during pregnancy
