What is Histrionic Personality Disorder?

Histrionic personality disorder belongs to Cluster B alongside borderline, narcissistic, and antisocial personality disorders. Diagnostic features include uncomfortable when not the center of attention, inappropriate sexually seductive behavior, rapidly shifting and shallow emotions, use of physical appearance to draw attention, impressionistic and lacking detail speech, theatrical exaggerated emotional expression, suggestibility, and considering relationships more intimate than they actually are.

Prevalence is 1.5 to 2 percent in general population, with female to male ratio of 4:1 (though clinical bias may inflate). Comorbidity with major depression, somatic symptom disorder, conversion disorder, and other personality disorders is common. Histrionic individuals typically present in crisis with relationship difficulties, depressive episodes, or dramatic somatic complaints rather than seeking treatment for the personality features themselves.

Treatment is primarily psychodynamic psychotherapy exploring transference patterns, schema-focused therapy targeting core emotional needs, and cognitive behavioral therapy addressing problematic thinking patterns. Group therapy provides feedback on interpersonal style. Medications target comorbid depression and anxiety. Therapy is challenging due to dramatic transference, sexualization of relationship with therapist, and resistance to introspection; long-term treatment with consistent boundaries achieves gradual improvement.
