What is Drug Interactions in Psychiatry: Pharmacokinetic and Pharmacodynamic Considerations?

Pharmacokinetic interactions affect drug absorption, distribution, metabolism (especially CYP450 enzymes) and elimination.

Pharmacodynamic interactions occur when drugs share or oppose mechanisms producing additive, synergistic or antagonistic effects.

Major CYP450 enzymes involved in psychotropic metabolism include CYP1A2, CYP2C19, CYP2D6 and CYP3A4.

Common dangerous interactions include serotonin syndrome, QT prolongation, hypertensive crisis with MAO inhibitors and bleeding risk with anticoagulants.

Genetic variations in metabolizing enzymes create poor, intermediate, normal and ultra-rapid metabolizer phenotypes affecting drug levels.

Symptoms

Serotonin syndrome with agitation, hyperreflexia, hyperthermia, tremor, diaphoresis and altered mental status.
Anticholinergic toxicity with confusion, dry mouth, constipation, urinary retention and tachycardia.
Sedation, ataxia and respiratory depression with combined CNS depressants.
Cardiovascular effects including QT prolongation, arrhythmias and hypotension.
Lithium toxicity from interactions with diuretics, NSAIDs or ACE inhibitors increasing lithium levels.

Risk Factors

Polypharmacy increases risk of significant drug interactions exponentially with each additional medication.
Elderly patients with reduced metabolic capacity and multiple comorbidities.
Hepatic or renal dysfunction affecting drug metabolism and elimination.
Genetic variants affecting CYP450 enzyme activity creating altered metabolizer status.
Concomitant use of substances of abuse, herbal remedies or supplements with potential interactions.

When to See a Doctor?

If you experience any of the following symptoms, seek medical attention promptly:

  • New unexpected symptoms after medication changes warrant urgent evaluation for drug interactions.
  • Suspected serotonin syndrome with autonomic instability requires immediate medical attention.
  • Severe sedation, confusion or altered mental status requires urgent medical evaluation.
  • Cardiovascular symptoms including syncope or palpitations may indicate dangerous interactions.
  • Therapeutic drug monitoring with abnormal levels guides management of suspected interactions.

Treatment Methods

01
Comprehensive medication review at each clinical encounter including over-the-counter, herbal and recreational substance use.
02
Use of validated drug interaction databases and pharmacist consultation for high-risk medication regimens.
03
Therapeutic drug monitoring for narrow therapeutic index medications (lithium, valproate, carbamazepine, clozapine).
04
Pharmacogenomic testing in selected patients with treatment failures or unusual adverse effects.
05
Multidisciplinary approach with psychiatry, pharmacy, primary care and relevant specialists along with patient education regarding self-management of drug interactions optimizes safety.
