What is Dissociative Disorders?

Dissociative disorders include dissociative identity disorder (DID, two or more distinct identity states with amnesia), dissociative amnesia (inability to recall traumatic events, may include fugue), and depersonalization-derealization disorder (persistent feeling of detachment from self or surroundings).

Etiology is rooted in early childhood trauma, especially repetitive abuse before age 9. Diagnosis uses Structured Clinical Interview for Dissociative Disorders (SCID-D) and Dissociative Experiences Scale. Comorbid PTSD, depression, self-harm, and substance use are common.

Treatment is phase-oriented: phase 1 stabilization with grounding techniques and safety planning, phase 2 trauma processing with prolonged exposure or EMDR cautiously titrated to dissociative capacity, phase 3 integration and reconnection. Hospitalization is reserved for safety crises. SSRIs and prazosin treat comorbid PTSD.
