What is Behavioral Symptoms of Dementia?

Behavioral and psychological symptoms of dementia (BPSD) encompass agitation, aggression, wandering, repetitive vocalizations, hallucinations, delusions (persecutory, theft, infidelity), depression, anxiety, apathy, sleep disturbance, and sundowning. They cause caregiver burden and institutionalization.

Assessment uses Neuropsychiatric Inventory (NPI), Cohen-Mansfield Agitation Inventory, and identification of triggers (pain, infection, constipation, environmental change, unmet needs). DICE approach (Describe, Investigate, Create, Evaluate) structures management.

First-line management is non-pharmacological: structured routine, reminiscence therapy, music therapy, exercise, environmental adaptation, and caregiver education. Pharmacotherapy reserves antipsychotics (risperidone, low-dose) for severe psychosis or aggression with documented risk-benefit. SSRIs treat depression and citalopram has evidence for agitation.
