What is Adjustment Disorder With Depressed Mood?

Adjustment disorder with depressed mood is defined in DSM-5 by the development of emotional or behavioral symptoms in response to an identifiable stressor occurring within 3 months of stressor onset.

Symptoms involve marked distress out of proportion to the severity of the stressor, or significant impairment in social, occupational, or other important areas of functioning, but do not meet full criteria for major depressive disorder.

Once the stressor or its consequences have terminated, symptoms do not persist for more than an additional 6 months.

Symptoms

Low mood, tearfulness, hopelessness related to a recent stressor
Loss of interest in usual activities — typically less pervasive than in MDD
Anxiety, worry about the precipitating event
Difficulty sleeping, especially with rumination about the stressor
Reduced concentration and motivation
Withdrawal from social interactions
Tearfulness during stressor-related discussions
Functional impairment proportionate to the precipitating event

Risk Factors

Recent significant life stressors: divorce, job loss, bereavement, illness, relocation
Pre-existing anxiety, mood, or personality vulnerabilities
Limited social support
History of childhood adversity
Multiple concurrent stressors
Female sex (slight predominance)
Cultural or religious dislocation
Adolescent or young adult age

When to See a Doctor?

If you experience any of the following symptoms, seek medical attention promptly:

  • Persistent depressed mood lasting more than 2 weeks after a stressor
  • Significant impairment in work, school, or relationships
  • Suicidal ideation or thoughts of self-harm (urgent)
  • Worsening symptoms despite resolution of stressor
  • Inability to perform daily activities
  • Symptoms emerging without an obvious trigger (suggests other disorder)
