What is Ventral Hernia Repair?

Ventral hernia describes any abdominal wall defect outside the inguinal region — incisional hernia (after previous surgery), primary epigastric, umbilical or Spigelian hernias.

Repair aims to reduce contents, close the defect and reinforce the abdominal wall with synthetic or biological mesh.

Modern techniques include open retro-rectus (Rives-Stoppa) repair, transversus abdominis release (TAR), laparoscopic intraperitoneal onlay mesh (IPOM) and robotic-assisted approaches.

Symptoms

Visible bulge in the abdominal wall (especially during straining, coughing)
Local pain and discomfort
Skin discolouration in incarcerated cases
Nausea, vomiting and obstipation in obstruction
Acute severe pain with strangulation (surgical emergency)
Cosmetic concerns and impaired core stability

Risk Factors

Previous abdominal surgery (incisional hernia)
Obesity and weight fluctuations
Chronic cough, smoking and constipation
Heavy lifting, occupational strain
Pregnancy and multiparity
Connective tissue disorders and aging

When to See a Doctor?

If you experience any of the following symptoms, seek medical attention promptly:

  • Painful, irreducible hernia (incarceration)
  • Skin discolouration with vomiting (strangulation, emergency)
  • Progressive enlargement affecting daily life
  • Recurrence after previous repair

Treatment Methods

01
Detailed preoperative imaging (ultrasound, CT) for defect size and intraabdominal volume
02
Optimisation of obesity, smoking, diabetes and abdominal wall strength
03
Open Rives-Stoppa retro-rectus or TAR repair for medium-large defects
04
Laparoscopic or robotic IPOM repair for small to medium hernias
05
Synthetic mesh in clean cases, biological or absorbable mesh in contaminated fields
06
Postoperative abdominal binder, gradual return to activity over six to eight weeks
