What is Ventilator-Associated Pneumonia (VAP)?

Ventilator-associated pneumonia (VAP) is the most common type of hospital-acquired pneumonia in intensive care units, developing 48 hours or more after the start of mechanical ventilation.

Most common pathogens include Pseudomonas aeruginosa, Acinetobacter baumannii, Staphylococcus aureus (often MRSA), and enteric Gram-negative bacilli. Multidrug resistance is frequent.

Diagnosis relies on clinical, radiological, and microbiological criteria. Empirical therapy is started initially and then de-escalated according to resistance and culture results.

Symptoms

New or increased purulent tracheal secretions
Fever or hypothermia
Leukocytosis or leukopenia
Increased oxygen requirement
New infiltrates on chest radiograph
Hemodynamic worsening
Increased respiratory rate and PEEP requirement

Risk Factors

Prolonged mechanical ventilation
Altered consciousness
Prior antibiotic use
Aspiration risk
Chronic lung disease
Long ICU stay
Immunosuppression

  • When purulent secretions increase
  • When hemodynamic instability develops
  • When sepsis criteria are met
