What is Complicated Varicella-Zoster in Adults?

Varicella-zoster virus (VZV), a member of the herpesvirus family, causes primary infection (varicella/chickenpox) and reactivation disease (herpes zoster/shingles). Adult primary varicella is more severe than pediatric disease, with higher rates of pneumonia (5-15%), hepatitis, encephalitis, and bacterial superinfection. Pregnancy increases maternal pneumonia risk and may cause congenital varicella syndrome (1-2% in first 20 weeks) or neonatal varicella.

Reactivation as herpes zoster occurs in 15-30% lifetime risk, increasing with age (50% by age 80) and immunosuppression. Complications include: postherpetic neuralgia (PHN, pain >90 days, in 10-15% of cases overall and >50% in elderly), herpes zoster ophthalmicus (V1 distribution, vision-threatening, requires urgent ophthalmologic evaluation), Ramsay Hunt syndrome (geniculate ganglion involvement with facial palsy and otalgia), zoster sine herpete, disseminated zoster (immunocompromised), and rare complications including encephalitis, myelitis, vasculopathy, and stroke.

Diagnosis is primarily clinical, supported by direct fluorescent antibody, PCR, or Tzanck smear. Treatment includes antivirals within 72 hours of onset (acyclovir, valacyclovir, famciclovir), with IV acyclovir for severe disease. Pain management combines NSAIDs, opioids, gabapentinoids, tricyclic antidepressants, and topical agents. Prevention with shingles vaccines (live-attenuated Zostavax or recombinant subunit Shingrix, with Shingrix preferred due to >90% efficacy) is recommended for adults ≥50 years.

Symptoms

Severe varicella with high fever and extensive rash
Varicella pneumonia (cough, dyspnea, hypoxia)
Dermatomal vesicular rash (zoster)
Severe burning, lancinating pain
Postherpetic neuralgia persisting >90 days
Eye involvement with ophthalmic zoster
Facial palsy with Ramsay Hunt syndrome

Risk Factors

Adulthood (more severe primary varicella)
Immunocompromised state (HIV, transplant, chemotherapy, biologics)
Older age (>50 years for zoster)
Pregnancy (severe varicella, congenital risk)
Smoking (varicella pneumonia)
Prior chickenpox (zoster reactivation)
Stress, trauma, recent surgery

When to See a Doctor?

  • Adult chickenpox with respiratory symptoms
  • Pregnancy with varicella exposure or symptoms
  • Vesicular rash in V1 distribution (ophthalmic zoster)
  • Vesicles with facial palsy or hearing loss
  • Severe pain or dermatomal rash within 72 hours
  • Persistent severe pain after rash resolution
  • Disseminated rash in immunocompromised patient
