What is Urethral Stricture?

Urethral stricture is the narrowing of the urinary canal due to scar tissue (spongiofibrosis) forming in the urethral wall. It is more common in men because the male urethra is longer and consists of several anatomical segments.

The most common causes include iatrogenic (catheterization, endoscopic intervention), trauma (straddle injury, pelvic fracture), inflammation (gonorrhea), lichen sclerosus, and congenital conditions.

The location, length, number, and etiology of the stricture determine treatment selection. Long-term outcomes of repeated dilation or internal urethrotomy are limited; urethroplasty is the gold standard treatment.

Symptoms

Weak and forked urinary stream
Difficulty voiding
Dribbling urine
Frequent urination
Sensation of incomplete bladder emptying
Recurrent urinary tract infections
Urinary retention in advanced cases

Risk Factors

History of urethral catheterization
Pelvic fracture history
Transurethral prostate surgery (TUR-P)
History of gonorrhea
Lichen sclerosus
Congenital urethral anomalies
Radiotherapy

When to See a Doctor?

If you experience any of the following symptoms, seek medical attention promptly:

  • Progressively difficult urination
  • Significantly weakened urinary stream
  • Recurrent urinary tract infections
  • Episodes of urinary retention
  • Hematuria and perineal pain

Treatment Methods

01
Stricture mapping with urethrography (RUG, VCUG)
02
Endoscopic internal urethrotomy (short, first-episode strictures)
03
Urethral dilation (temporary solution)
04
Anastomotic urethroplasty (short bulbar strictures)
05
Patch (buccal mucosa) urethroplasty: long and recurrent strictures
06
Suprapubic cystostomy (in emergency urinary retention)
