What is Urethral Carcinoma Treatment?

Urethral carcinoma is a rare cancer that arises from the epithelium of the urethra. It is four times more common in women than men. Histological subtypes include squamous cell carcinoma (54%), urothelial carcinoma (34%) and adenocarcinoma (12%).

It most often arises in the bulbar urethra in men and the distal urethra in women. Treatment is selected by location, stage and histology, and organ-sparing surgery may be possible for distal tumours.

Locally advanced and metastatic disease is managed with neoadjuvant chemoradiation and surgery. Prognosis depends on stage and lymph node status; five-year survival in early-stage disease is 50-70%.

Symptoms

Urethral bleeding or bloody discharge
Weak urinary stream or obstruction
Palpable urethral mass or induration
Pelvic or perineal pain
Recurrent urinary tract infections
Urinary retention
Inguinal lymphadenopathy as a sign of metastasis

Risk Factors

HPV infection
History of urethral stricture
Chronic infection and irritation
History of sexually transmitted infection
Lichen sclerosus
Prior radiotherapy
