What is Ureteroscopic Holmium Laser Lithotripsy?

Ureteroscopic holmium laser lithotripsy uses a semirigid or flexible ureteroscope to access ureteric or kidney stones, which are then fragmented with holmium laser energy and retrieved with a basket catheter.

Holmium laser can effectively fragment all stone compositions (calcium oxalate, uric acid, cystine, struvite). Both dusting and fragmentation techniques are used.

Flexible ureteroscopy can reach all calyces, including the lower pole. Stone-free rates are 95-97% for ureteric stones and 85-90% for renal stones. The procedure can often be performed as a day-case.

Symptoms

Severe flank pain (renal colic)
Hematuria (blood in urine)
Nausea and vomiting
Frequent and burning urination
Urinary tract infection
Hydronephrosis (on ultrasound)

Risk Factors

Recurrent stone disease
Metabolic stone disease (hypercalciuria, hyperoxaluria)
Inadequate fluid intake
Excessive salt and animal-protein intake
Ureteric stricture
Obesity and diabetes

When to See a Doctor?

If you experience any of the following symptoms, seek medical attention promptly:

  • Severe flank pain with anuria
  • Stone obstruction with fever (obstructive pyelonephritis)
  • Ureteric stone larger than 6 mm that fails to pass
  • Stones resistant to ESWL
