What is Ureterocele Incision Therapy?

A ureterocele is a cystic dilation of the segment of the ureter that opens into the bladder (intravesical segment). It is a congenital anomaly that can obstruct urinary flow and lead to hydronephrosis, infection and stone formation.

There are two types: intravesical (orthotopic) and ectopic ureteroceles. Ectopic ureteroceles extend toward the bladder neck or urethra and are often associated with the upper pole ureter of a duplex collecting system.

The first-line therapy is endoscopic incision, in which the ureterocele wall is cut with laser or electrocautery through a cystoscope to provide decompression. Success rates are 70-90%.

Symptoms

Recurrent urinary tract infections (in children)
Hydronephrosis on ultrasonography
Febrile urinary infection
Dilation seen on prenatal ultrasonography
Bladder outlet obstruction (large ectopic ureterocele)
Urinary incontinence (ectopic ureterocele in females)

Risk Factors

Duplex collecting system (double ureter)
Female sex (4-7 times more common)
Congenital urinary tract anomalies
Non-functioning upper pole renal segment
Recurrent urinary infection
Stone formation

When to See a Doctor?

If you experience any of the following symptoms, seek medical attention promptly:

  • Recurrent febrile urinary infections in children
  • Urinary tract dilation on prenatal ultrasonography
  • Ureterocele or hydronephrosis on ultrasound
  • Follow-up for reflux developing after incision

Treatment Methods

01
Endoscopic ureterocele incision (laser or electrocautery)
02
Upper pole heminephrectomy (when there is a non-functioning segment)
03
Ureteral reimplantation (if reflux develops)
04
Ureterocele excision plus bladder reconstruction
05
Double-J stent placement (temporary drainage)
06
Long-term follow-up with VCUG and ultrasound
