What is Two-Stage Anterior Urethral Reconstruction?

Two-stage anterior urethral reconstruction (Bracka technique) is a procedure used in long anterior urethral strictures, hypospadias failures and lichen sclerosus-related strictures.

Stage-1 places a buccal mucosa graft on the urethral plate and waits for it to mature; stage-2 (typically after 6 months) tubularises the matured graft to form the neo-urethra. It offers higher success than single-stage urethroplasty in particularly severe stricture cases.

Symptoms

Recurrent anterior urethral stricture
Long stricture (>5 cm)
Hypospadias failure (multiple operation history)
Lichen sclerosus stricture
Severe periurethral fibrosis
Recurrence after single-stage urethroplasty

Risk Factors

Long-segment stricture (>5 cm)
Lichen sclerosus (BXO)
Multiple urethrotomy history
Hypospadias failure
Severe penile chordee
Local poor tissue conditions
