What is Tuberculosis Advanced Management?

Mycobacterium tuberculosis spreads via airborne droplets and primarily affects lungs but can disseminate to lymph nodes, bones, central nervous system, and other organs. After exposure, 90 percent develop latent infection that may reactivate, while 10 percent progress to active disease, especially in HIV co-infection or immunosuppression.

Diagnosis combines clinical assessment with chest imaging, sputum acid-fast bacilli smear, mycobacterial culture (gold standard, 2 to 6 weeks), nucleic acid amplification tests like Xpert MTB/RIF for rapid diagnosis with rifampicin resistance detection, and tuberculin skin test or interferon-gamma release assay for latent infection screening.

Standard regimen for drug-susceptible TB is 4 months of isoniazid, rifapentine, moxifloxacin, and pyrazinamide (modern shortened regimen) or traditional 6-month isoniazid plus rifampin with pyrazinamide and ethambutol for first 2 months. Multidrug-resistant TB requires 9 to 18 month regimens including bedaquiline, linezolid, and pretomanid. Latent infection prophylaxis uses 3 months isoniazid-rifapentine.

Symptoms

Chronic cough longer than 3 weeks with sputum
Hemoptysis (blood in sputum)
Weight loss and night sweats
Low-grade fever and fatigue
Lymphadenopathy with extrapulmonary disease

Risk Factors

Recent exposure to active TB case
HIV infection or immunosuppression
Diabetes mellitus and chronic kidney disease
Silicosis and tobacco use
Healthcare worker or congregate settings
