Tonsillitis

What is Tonsillitis?

Tonsillitis is acute or chronic inflammation of the palatine tonsils, part of Waldeyer's lymphoid ring at the entrance to the upper aerodigestive tract. It commonly affects children and young adults, especially during winter and early spring.

Most cases (about 70-80%) are viral, caused by adenovirus, EBV, influenza, parainfluenza or rhinovirus. Group A beta-hemolytic streptococcus (GABHS) accounts for 15-30% in children and warrants antibiotic therapy to prevent acute rheumatic fever, post-streptococcal glomerulonephritis and peritonsillar abscess.

Diagnosis is clinical, supported by Centor or McIsaac scoring, rapid antigen detection tests and throat culture. Treatment includes adequate hydration, analgesics, antipyretics and, when GABHS is confirmed, penicillin or amoxicillin. Tonsillectomy may be considered for recurrent tonsillitis (Paradise criteria), peritonsillar abscess or obstructive symptoms.

Childhood and adolescent age
Crowded settings (schools, dormitories)
Recurrent upper respiratory infections
Immunodeficiency
Cigarette smoke exposure
Close contact with strep carriers
Atopic background

  • Severe sore throat with high fever
  • Inability to swallow saliva or fluids
  • Trismus or muffled voice
  • Unilateral neck swelling and severe pain
  • Recurrent tonsillitis episodes per year
