What is Thulium Fiber Laser Prostate Enucleation (ThuLEP)?

Thulium Fiber Laser Prostate Enucleation (ThuLEP) is an endoscopic procedure used in symptomatic benign prostatic hyperplasia. Unlike traditional TURP, the obstructive adenomatous tissue is anatomically separated from the prostate capsule with thulium laser energy and removed entirely with a morcellator.

Compared with TURP and HoLEP it allows precise dissection, lower bleeding risk and faster catheter removal; it is also a safe alternative for patients on anticoagulants and those with very large prostates.

Postoperative urinary continence and sexual function are preserved at high rates; transient urinary urgency, urge incontinence and irritative symptoms may occur in some patients.

Symptoms

Postoperative haematuria
Urinary urgency and frequency
Transient urge incontinence
Burning during urination
Urinary tract infection
Bladder neck contracture (rare)
Retrograde ejaculation

Risk Factors

Symptomatic benign prostatic hyperplasia
Large prostate (>80 mL)
Failure of medical therapy
Recurrent acute urinary retention
Recurrent urinary tract infection due to BPH
Patient on anticoagulant therapy
Bladder stones associated with BPH
