What is TEP (Totally Extraperitoneal Inguinal Hernia Repair)?

Totally Extraperitoneal (TEP) repair is a minimally invasive laparoscopic inguinal hernia technique that places mesh in the preperitoneal space without entering the peritoneal cavity, theoretically reducing risks of bowel injury, adhesions, and visceral complications.

Through a sub-umbilical incision and balloon dissection, the surgeon develops the preperitoneal space, identifies and reduces the hernia sac, and places a polypropylene or composite mesh covering the myopectineal orifice (direct, indirect, and femoral defects).

Compared to TAPP (transabdominal preperitoneal), TEP avoids peritoneal entry but has a steeper learning curve. Both offer faster recovery and lower chronic pain compared to open Lichtenstein repair, particularly in bilateral or recurrent hernias.
