What is TEP Hernia Repair?

Totally Extraperitoneal (TEP) repair is a laparoscopic approach that creates a balloon-dissected working space below the rectus muscle and over the preperitoneal fascia without breaching the peritoneum.

Trocars are placed in the preperitoneal plane, the spermatic cord and inguinal anatomy are dissected, the hernia sac is reduced, and a polypropylene mesh covers the entire myopectineal orifice; fixation is often unnecessary in primary cases.

Avoiding the abdominal cavity reduces the risk of bowel injury and adhesions and shortens postoperative ileus. Pain is mild, hospital stay is 1 day, and return to work occurs in 1-2 weeks; learning curve is steeper than TAPP.

Risk Factors

Suitable for general anesthesia
Adequate laparoscopic experience
Absence of large scrotal hernia (relative)
No prior preperitoneal surgery
No severe coagulopathy
Tolerance to pneumoperitoneum
Avoidance of intra-abdominal entry
