What is Surgical Treatment of Mesenteric Ischaemia?

Acute mesenteric ischaemia is most often caused by embolus to the superior mesenteric artery; chronic forms result from atherosclerotic stenosis. Diagnosis requires high clinical suspicion, CT angiography and laboratory investigation.

Surgical options include open embolectomy, mesenteric bypass, retrograde mesenteric stenting and endovascular thrombolysis. Necrotic bowel is resected with primary anastomosis or stoma.

Postoperative second-look laparotomy at 24-48 hours assesses bowel viability. Mortality is 50-80% and is closely related to early intervention; poor prognostic factors are advanced age, sepsis and extensive resection.
