What is Surgical Biliary Tract Reconstruction?

Indications for biliary reconstruction include iatrogenic bile-duct injury, choledochal cysts, malignancy (cholangiocarcinoma, pancreatic cancer), chronic stricture and primary sclerosing cholangitis.

The most common technique is Roux-en-Y hepaticojejunostomy. Choledochojejunostomy, choledochoduodenostomy and intrahepatic bile-duct anastomoses are used in selected cases.

Preoperative imaging includes MRCP, ERCP and CT. Postoperative complications are anastomotic leak, stricture, cholangitis and stone formation; long-term follow-up is required.
