What is Sublay Retrorectus Hernia Repair?

Sublay retrorectus repair was developed by Rives and Stoppa. The mesh is placed below the hernia defect, behind the rectus muscle and in front of the posterior rectus sheath or peritoneum.

Often called the modified Rives-Stoppa repair, the technique relies on intraabdominal pressure pressing the mesh against the posterior rectus sheath, reinforcing the hernia area.

Use of porous polypropylene mesh, fixation with 5 to 6 cm overlap beyond the defect edges, and avoidance of mesh-bowel contact are the principal technical requirements; recurrence rates with this technique are around 5 percent.

Symptoms

Medium-sized ventral incisional hernia
Recurrent primary umbilical hernia
Large epigastric hernia
Small midline eventration
Posttraumatic abdominal wall reconstruction
Multidefect ventral hernia
Re-repair after mesh failure
Hernia coexisting with diastasis recti

Risk Factors

Chronic seroma
Hematoma in the retromuscular space
Mesh infection in 3 to 5 percent
Inferior epigastric vessel injury
Anterior rectus sheath weakening
Wound dehiscence and infection
Recurrence in 5 to 8 percent
Long-term chronic pain
