What is Sporotrichosis (Rose Gardener's Disease)?

Sporotrichosis is a chronic subcutaneous fungal infection caused by Sporothrix schenckii and related species. The organism is found in soil, plant debris and rose thorns and is inoculated into the skin through trauma.

It is classically known as 'rose gardener's disease'. The lymphocutaneous form is most common, beginning with a nodule at the inoculation site followed by chains of nodules along the lymphatic vessels.

Less common forms are fixed cutaneous, disseminated cutaneous, pulmonary and systemic. Systemic involvement is more frequent in immunocompromised patients. Treatment is itraconazole; severe disease requires amphotericin B.

Symptoms

Small painless papule or nodule at the entry site
Chain of nodules along lymphatic channels
Ulceration of nodules
Chronic, slowly progressive course
Regional lymph node enlargement
Fever and myalgia in systemic disease
Cough and sputum in pulmonary forms

Risk Factors

Gardening and floriculture
Forestry work
Rose thorn pricks and similar trauma
Living in endemic regions
Immunosuppression
Alcohol use disorder

When to See a Doctor?

If you experience any of the following symptoms, seek medical attention promptly:

  • Persistent skin nodule following gardening activity
  • Spread of nodules along a lymphatic line
  • Ulcerated, non-healing skin lesions
  • Associated systemic symptoms
  • Any new skin lesion in a patient on immunosuppression
