What is Spigelian Hernia?

Spigelian hernia is an interparietal abdominal wall hernia that protrudes through the aponeurosis of the transversus abdominis between the rectus muscle and the semilunar line, accounting for 0.12-2% of all abdominal hernias.

It is most often seen in women aged 40-70, located 0-6 cm above and lateral to the umbilicus, and presents with an intermittent palpable mass and dull abdominal pain. Because the sac stays beneath the external oblique aponeurosis, swelling is rarely visible.

Diagnosis is confirmed by abdominal ultrasound or CT. Because the spigelian defect is typically narrow and small (1-2 cm), the risk of incarceration and strangulation is high (15-30%) and surgical repair is recommended in all cases.

Symptoms

Lateral abdominal wall pain
Intermittent palpable mass
Pain that increases with movement
Discomfort while bending
Sudden severe pain (incarceration)
Nausea and vomiting (strangulation)
Localized tenderness in the lower quadrant

Risk Factors

Female sex
Age 40-70
Obesity
Multiparity
Chronic cough or COPD
Constipation
Connective tissue disease
