What is Sphenoid Sinusitis?

The sphenoid sinus lies deep in the skull base and shares walls with the optic nerve, cavernous sinus, internal carotid artery and pituitary gland, so its inflammation can mimic neurologic disease and may cause serious complications.

Causes include viral, bacterial or fungal infection, mucocele, anatomic obstruction or extension from neighbouring sinuses. Diagnosis often requires CT or MRI as physical exam and rhinoscopy may be normal.

Isolated sphenoid sinusitis is uncommon (around 3% of all sinusitis) but its recognition is critical because untreated disease can progress to cavernous sinus thrombosis, meningitis or visual loss.

Symptoms

Deep vertex or retro-orbital headache
Occipital pain
Photophobia
Visual disturbance
Cranial nerve palsy
Postnasal drip
Fever in acute infection

Risk Factors

Allergic rhinitis
Anatomic obstruction of the sphenoid ostium
Immunosuppression
Diabetes mellitus
Recent upper respiratory infection
Nasal polyps
Skull base trauma or surgery

When to See a Doctor?

  • Severe headache unresponsive to analgesics
  • Visual disturbance or diplopia
  • Cranial nerve deficit
  • Persistent fever
  • Meningitis suspicion
  • Worsening symptoms despite antibiotics
